The Day Hospice is situated at the front of Florence Nightingale Hospice. We are open Monday, Wednesday and Thursday 10.00am - 3.00pm.
Our assessment day is held on Tuesdays and anyone with specialist palliative care needs to be referred, via an appointment system. The assessment takes place in the morning and is led by a team consisting of a Doctor, Nurse, Creative Therapist and Chaplain. A family member or carer/friend may also attend with you.
Appointments are also available with the nurse on Tuesday afternoons: if you have any concerns there is also a facility for you to drop in on any of those afternoons.
Following the assessment you may be offered a 12 or 24 week placement. If you are offered a placement you will be allocated a specific day and we will try and organise a volunteer driver to bring you in and out of the Hospice.
Lunch and drinks will be provided whilst you are here. Please bring along any medicines you will need during your time with us.
A plan of care will be discussed with you, and agreed goals planned. Whilst in the Day Hospice, regular review will take place. Discharge dates, when appropriate, will also be discussed and planned with you.
The planned care could include any aspects of your disease that are of particular concern to you and/or your family. It can also be important for you and your family to have some ‘time out’. We work very closely with your GP and Community Team.
